Full answer and how we checked it

For nonprofit and public rural hospitals, 2026 grant money comes from four main places: HRSA programs passed through your State Office of Rural Health (SHIP and Flex), HRSA grants you can apply for directly (network awards of $300,000 to $500,000), USDA Community Facilities grants of up to 75% of project cost, and private foundations that report giving on IRS Form 990-PF. CMS's Rural Health Transformation Program adds $10 billion a year to states for fiscal years 2026 through 2030, which reaches hospitals through state plans.

Data note: Rural hospital funding counts are Grantsights analysis of IRS Form 990-PF grant records tagged to health, by recipient state, pulled October 29, 2026, drawn mostly from tax year 2023 to 2025 filings. Federal details come from the eCFR text of 42 CFR Part 485 and 7 CFR Part 3570, Grants.gov (opens in a new tab) records for HRSA programs, and CMS's Rural Health Transformation Program page.

Across the US, these foundations gave 282,323 grants totaling $48,801,068,047.

Historical giving does not establish an open application or eligibility for your organization. For-profit operators should check each foundation's applicant rules before buying.

The full list names all 43,922, state by state: what each gave, to whom, for what, and a link to its filings. One payment, no subscription.

$29 keeps this list for good. $99 opens all nine causes, including churches and ministries, in every state for a year.

  • Foundation data from the foundations' own public tax returns (IRS Form 990-PF), not government grants. We never promise funding.
  • Paid by card through Stripe, with an emailed receipt (refund details in our terms).
  • For comparison, full foundation databases are subscriptions: Candid Premium is $1,199 a year (nonprofit price, billed annually) and Instrumentl starts at $299 a month (billed annually). This list is $29, once.

If you're the CEO, CFO, foundation director or board chair of a critical access hospital or another small rural hospital, this guide explains which money flows through your state, which you can apply for yourself, and how to find the local foundations that already fund rural hospitals' equipment and buildings.

Rural hospital source review, Grantsights, October 29, 2026: Grantsights checked the critical access hospital conditions of participation, the SHIP and Flex notices, three current HRSA rural health listings, USDA Community Facilities grant limits and the CMS Rural Health Transformation overview, then reviewed hospital grants named in 990-PF filings. Where official sources stop: HRSA explains how SHIP money moves; it doesn't name the family trust in your county that bought the last CT scanner.

The 5 Routes to Grant Money for a Rural Hospital

Rural hospitals have more federal grant options than most nonprofits, but the biggest ones don't accept applications from hospitals. Here's how the routes compare:

RouteProgramsWho appliesTypical use
State Office of Rural Health pass-throughSHIP, FlexThe state office or a Governor-designated entityHardware, software, training, quality improvement
Direct HRSA applicationsRural Health Network Development, Small Health Care Provider Quality ImprovementHospitals and partnersNetworks, quality programs
USDA Rural DevelopmentCommunity Facilities grants and loansPublic bodies and nonprofits in towns of 20,000 or fewerBuildings, equipment
State transformation plansCMS Rural Health Transformation ProgramStatesSet by each state's approved plan
Private foundationsFamily, community and health conversion foundationsThe hospital or its foundationEquipment, expansions, service lines

The honest summary: the state office is your most important federal relationship, and your hospital foundation is your most important private one.

Critical Access Hospital Rules That Shape Eligibility

Many rural grant programs are built around the critical access hospital (CAH) designation, so know the rules. According to 42 CFR 485.620 (opens in a new tab), a CAH maintains no more than 25 inpatient beds, which can be used for inpatient or swing-bed services, and its acute inpatient stays can't exceed an annual average of 96 hours per patient.

Location matters too. Under 42 CFR 485.610 (opens in a new tab), a CAH must be more than a 35-mile drive on primary roads from a hospital or another CAH, or a 15-mile drive in mountainous terrain or areas with only secondary roads, unless the state certified it as a necessary provider before January 1, 2006.

SHIP and Flex: HRSA Money That Comes Through Your State

SHIP. According to Grants.gov (opens in a new tab), the most recent Small Rural Hospital Improvement Program notice, HRSA-23-033 (opens in a new tab), estimated about $20.7 million for 46 awards. The State Office of Rural Health is the official award recipient and acts as fiscal intermediary for all eligible hospitals in its state: it applies on their behalf, receives the federal funds, verifies hospital eligibility and makes awards to the hospitals. SHIP supports small rural hospitals in meeting value-based payment and care goals through purchases of hardware, software and training.

Flex. The Medicare Rural Hospital Flexibility Program's latest notice on Grants.gov (opens in a new tab), HRSA-24-002 (opens in a new tab), estimated $30 million across about 45 awards. HRSA accepts one application per state, from a Governor-designated organization. The program supports CAHs in quality improvement and reporting, performance improvement and benchmarking, helps facilities seeking CAH designation, and supports rural EMS.

If you don't know your state office contact, find it before the next SHIP cycle. An eligible hospital that misses the state office's internal deadline can get nothing that year.

HRSA Grants a Rural Hospital Can Apply For Directly

According to Grants.gov (opens in a new tab), two current Federal Office of Rural Health Policy programs accept hospitals as applicants:

ProgramStatus on October 29, 2026Award sizeNotes
Rural Health Network Development (HRSA-27-051) (opens in a new tab)Forecast for FY 2027$300,000 to $500,000; about 23 awardsEligible applicants include hospitals and rural emergency hospitals; two tracks, including one for networks of independent rural hospitals and clinics
Small Health Care Provider Quality Improvement (HRSA-26-046) (opens in a new tab)Closed August 6, 2026Up to $250,000; about 20 awardsRural public or nonprofit providers such as critical access hospitals; focus on quality data and chronic disease outcomes

The Network Development forecast's advancement track is built for independent rural hospitals and clinics working together, to help small providers compete as health care consolidates. If your hospital is independent, that's a strong fit. For HRSA scoring in more depth, see the HRSA grants guide.

USDA Community Facilities: Buildings and Equipment

For construction and equipment, USDA Rural Development's Community Facilities program is the federal source. According to 7 CFR 3570.62 (opens in a new tab), grants of up to 75% of the cost of developing essential community facilities, including health services operated on a nonprofit basis, can supplement other financing.

The grant share is tiered. According to 7 CFR 3570.63 (opens in a new tab), the 75% level applies to rural communities of 5,000 or fewer with median household incomes below the higher of the poverty line or 60% of the state nonmetropolitan median, and the scale steps down through 55% and 35% to 15% for communities of up to 20,000 people with higher incomes. For most hospitals, Community Facilities loans do the heavy lifting and grants fill smaller gaps. The USDA Community Facilities grants guide explains the tiers.

The Rural Health Transformation Program

CMS's Rural Health Transformation Program (opens in a new tab) allocates $50 billion to approved states over fiscal years 2026 through 2030, $10 billion each year, under Section 71401 of Public Law 119-21. Only the 50 states can receive awards. Half of each year's money is split equally among approved states, and CMS allocates the other half using factors such as rural population and the share of rural health facilities.

Hospitals don't apply to CMS. Each state decides how its plan uses the money, including payments to providers and support for rural facilities. Ask your state health agency and your office of rural health how your state's plan reaches hospitals and what your hospital must do to take part.

Which Foundations Fund Rural Hospitals: What 990-PF Filings Show

Grantsights counted 1,428 health grants to Montana recipients and 642 to Wyoming recipients in 990-PF filings. According to IRS rules, private foundations list every grant on IRS Form 990-PF (opens in a new tab), with the recipient and purpose, so rural hospital donors are visible.

Hospitals fall under Grantsights' health cause, with clinics and health charities. The counts below are for that whole health pool, from records pulled October 29, 2026:

Small-town hospital grants named in the filings

These tax year 2025 grants went to hospitals and hospital foundations in small towns:

  • The Blue Cross and Blue Shield of Mississippi Foundation gave the South Sunflower County Hospital Foundation in Indianola, Mississippi $5,300,000 for a women's health center.
  • The J. Willard and Alice S. Marriott Foundation gave Huggins Hospital in Wolfeboro, New Hampshire $1,000,000 for MRI equipment and renovations.
  • The Vernon and Rowena Griffith Foundation gave Sheridan Memorial Hospital in Wyoming $250,000 to support hospital functions.
  • The Joslin-Needham Family Foundation (opens in a new tab) gave East Morgan County Hospital in Brush, Colorado $216,753 for equipment.
  • The Brenden Family Foundation gave Santiam Hospital and Clinics in Stayton, Oregon $200,000 to remodel a women's clinic.
  • The Seedlings Foundation gave the Grant Regional Health Center Foundation in Lancaster, Wisconsin $200,000.
  • The James W. and Ella B. Forster Trust gave Merrill Pioneer Community Hospital in Rock Rapids, Iowa $183,685 for a facility expansion.
  • The Robert and Mary Jane Smith Foundation gave Steele Memorial Medical Center in Salmon, Idaho $150,000 for health care.
  • The George Polivka Trust gave Gundersen Moundview Hospital in Friendship, Wisconsin $140,000.
  • The Walter and Virginia Hager Foundation gave the Mobridge Region Hospital Foundation in South Dakota $25,000 to support its local hospital.

One hospital in Iowa shows the pattern: the Merrill Pioneer grant paid for a defined facility expansion, and nearly every grant above names a specific project or a local hospital by name. Treat the trusts that already give to your hospital as your first prospects, and give them a named project.

Get the health funder list for your state

Hospitals and hospital foundations can use the Grantsights health funder list, which shows foundations that reported health grants on their own 990-PF filings, by state, with the ones that take requests listed first. Recipients and purpose lines show which funders gave to a hospital, clinic or EMS service in your region. The preview for each state is free. The full health list is a single $29 payment covering every state, the $99 plan includes every cause list for a year, and Grantsights refunds either within 30 days.

What HRSA Doesn't Tell You About Funding a Rural Hospital

  1. Your hospital foundation is the grant applicant many funders prefer. A separate 501(c)(3) foundation can hold endowments and memorial gifts. Several grants above went to a hospital foundation rather than the hospital.
  2. Equipment is the easiest private ask. Funders understand a scanner, an ambulance bay or a birthing suite. Price it, show the use volume and name the room.
  3. Public hospitals benefit from a charitable partner. County or district hospitals can accept gifts, but some family foundations prefer to give to a 501(c)(3) hospital foundation.
  4. The state office relationship pays for itself. SHIP, Flex and many state-run programs move through one office. A short call each year keeps you on its list.

Common Mistakes in Rural Hospital Grant Requests

  • Applying to HRSA for SHIP directly. A common mistake is looking for a SHIP application on Grants.gov (opens in a new tab). Hospitals apply through the State Office of Rural Health.
  • No volume or outcome data. Funders want inpatient days, emergency visits, births, distance to the next hospital and payer mix. A request without them is a red flag.
  • Asking local trusts for operating losses. Family trusts fund things they can see. Present losses with a plan and ask for the equipment or service line that fixes them.
  • Underestimating USDA timelines. Community Facilities projects can take months of environmental review and financing work. Start early.
  • Lapsed SAM registration. HRSA and USDA need an active registration. See how to renew SAM.gov registration.

FAQ: Common Questions

How does a rural hospital get SHIP or Flex money? Through the State Office of Rural Health or the state's Governor-designated Flex entity, not directly from HRSA.

What qualifies a hospital as a critical access hospital? Among other conditions, 25 or fewer inpatient beds, a 96-hour average stay and the 35-mile distance rule.

Are there HRSA grants a rural hospital can apply for directly? Yes. The FY 2027 Rural Health Network Development forecast lists $300,000 to $500,000 awards with hospitals eligible.

What is the Rural Health Transformation Program? A CMS program giving $50 billion to approved states over fiscal years 2026 through 2030; hospitals benefit through state plans.

What do private foundations give rural hospitals? Equipment, buildings and service lines, such as the $216,753 for equipment East Morgan County Hospital received.
